Job Description
Join Liberty Healthcare Management as a Hospice Clinical Manager - RN and make a meaningful difference in the lives of patients and families facing terminal illness. As part of our team, you will coordinate case management, supervise care staff, and ensure the highest standards of care and compassion.
Responsibilities
- Coordinate case management of all patient services.
- Supervise and support assigned care staff.
- Provide direct patient care including Oasis care planning.
- Ensure compliance with hospice/homecare regulations.
- Maintain timely and accurate documentation of care.
Requirements
- Current RN licensure in North Carolina.
- Minimum 3 years of clinical nursing experience.
- Preferably with hospice experience.
- Strong communication and critical thinking skills.
- Computer literacy and CPR certification required.
