INPATIENT CASE MANAGER (RN)

Job Description

We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position in Randallstown, MD at Northwest Hospital offers a sign-on bonus potential of $15,000. The role involves providing discharge planning coordination and intervention in collaboration with the clinical team and medical provider. The ideal candidate will promote patient wellness, improve care outcomes, ensure efficient health service utilization, and minimize payment denials for a patient population with complex health needs.

Requirements

• Bachelor's degree and BSN from CCNE accredited schools.
• Working knowledge of theory and practice within a specialized field.
