Job Description

We are currently seeking a compassionate and experienced Home Care Registered Nurse (RN) to join Liberty Healthcare Management's team. In this role, you will actively participate in the interdisciplinary care planning process, provide professional nursing services, promote favorable outcomes through collaborative practice, and ensure accurate documentation of home health services.

Responsibilities

• Actively participate in care planning process.
• Provide professional nursing services based on established care plans.
• Promote favorable outcomes through collaborative practice.
• Document services accurately and thoroughly in compliance with regulations.
• Demonstrate effective interpersonal relationships and accountability.

Requirements

• Completion of nursing education from an approved school.
• Current RN licensure in the state.
• Clinical nursing experience preferred.
• Knowledge of medical-surgical principles and practices.
• Excellent clinical skills and communication abilities.
• Computer proficiency and critical thinking skills.
• CPR certification.
