HOSPICE MEDICAL SOCIAL WORKER

Job Description

We are currently looking for a compassionate and experienced Hospice Medical Social Worker to join our team at Liberty Hospice. As a crucial member of our staff, you will provide direct patient care services, conduct clinical assessments, and develop individualized care plans that focus on the psycho-social well-being of our patients and their families. This position will involve coordinating with various healthcare services to ensure the best possible care for our patients and their families.

Responsibilities

• Provide direct patient care services and document patient care data.
• Conduct clinical assessments to develop individualized care plans focused on patient treatment goals.
• Apply psycho-social principles to identify factors affecting medical treatment and outcomes.
• Coordinate with agency personnel and other services to optimize patient care.
• Offer bereavement support and grief care to patients and caregivers.

Requirements

• Hold a Master of Social Work degree from a Council on Social Work Education accredited school.
• Have a minimum of one year of practical experience (preferably in home health/hospice care).
• Possess strong interpersonal skills for effective communication in a multidisciplinary environment.
• Be willing to travel and work as needed.
• Complete a minimum of twelve hours of continuing education annually.
