Social Worker

Job Description

Northwest Kidney Centers is seeking a dedicated Social Worker to join our team in a full-time, variable position. This role involves providing social work intake, patient advocacy services, and coordinating social services to ensure comprehensive care for patients. As part of the interdisciplinary team, the Social Worker assesses each patient's social service needs, develops individualized care plans, and contributes to ongoing quality improvement initiatives.

Responsibilities

• Conduct social work intake and patient advocacy services.
• Coordinate social services with healthcare professionals, patients, and families.
• Provide patient education, counseling, and support services.
• Participate in quality assessment performance improvement activities.

Requirements

• Master of Social Work (MSW) degree.
• Current Washington Social Worker License.
• Valid WA Driver's license, reliable transportation, and car insurance.
• Previous experience in medical social work (preferred).
