Primary Care Physician (Bilingual Spanish)

Job Description

We are looking for a compassionate and dedicated Primary Care Physician (Bilingual Spanish) to join Esperanza Health Centers. In this role, you will provide primary medical care services to older adult participants, emphasizing preventive care, and healthy living within the Program of All-Inclusive Care for the Elderly (PACE).

Responsibilities

• Manage primary care practice at the PACE center, participants' homes, and post-acute facilities.
• Collaborate with the Interdisciplinary Team for individualized care plans.
• Participate in quality improvement activities and utilization management.
• Ensure continuity of care by communicating with partner providers.

Requirements

• Bilingual in English and Spanish.
• Board certification in primary care, geriatrics specialization preferred.
• State of Illinois physician license required.
• Valid driver's license and automobile insurance.
