Responsibilities:

• Provide in-home care to patients post-discharge from medical facilities
• Collaborate with physicians, supervise LPNs, CNAs, and aides in patient care
• Conduct patient assessments, document progress, and communicate changes to physicians
• Educate patients on care plans, medications, therapy, diet, and exercise
• Train family members on patient care plans and home safety
• Ensure treatment goals are achieved and facilitate discharge processes

Requirements:

• Minimum of 1 year of acute care experience, home healthcare experience preferred
• Oasis experience required
• Graduate of an accredited nursing program with an active RN license in FL
• CPR certification
• Familiarity with state and federal home health regulations
• Proficiency in nursing specialty, strong communication skills, and compassion
