INPATIENT CASE MANAGER (RN)

Job Description

We are looking to add a dedicated and compassionate Inpatient Case Manager (RN) to our team at LifeBridge Health. As an Inpatient Case Manager, you will collaborate with the clinical team and medical providers to ensure optimal discharge planning coordination and intervention for patients with complex health needs. The role focuses on promoting patient wellness, enhancing care outcomes, utilizing health services efficiently, and reducing payment denials.

Responsibilities

• Coordinate discharge planning in collaboration with clinical team and medical provider.
• Promote patient wellness and improved care outcomes.
• Ensure efficient utilization of health services.
• Minimize denials of payment for patient services.

Requirements

• Bachelor's degree with specialized field knowledge.
• BSN required from CCNE accredited schools.
• Experience working as a Utilization Review Nurse, Clinical Care Coordinator, Patient Care Integrator, or RN Case Manager will be considered.
