Job Description

Job Responsibilities and Duties:
- Assess the needs of persons served on waivers.
- Coordinate resources and services for individuals supported.
- Develop service plans and ensure service delivery meets client needs.
- Attend meetings with interdisciplinary teams and maintain client case notes.
Requirements
- Bachelor's degree in social work or related field.
- Experience in social work/case management.
- Valid driver's license and reliable transportation.
- Clear background checks.
- LSW or BOSW licensure preferred.
Salary
$49,000 - $51,600 per year for 40 hours/week.
