Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a valuable member of our Social Work Department, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face, including adjustment to institutional living, financial concerns, family dynamics, medical procedures, aging-related issues, discharge planning, and end-of-life counseling.
Responsibilities
- Deliver clinical social work services and psychosocial assessments to patients and families.
- Address common challenges such as adjustment issues, financial concerns, and family relationships.
- Assist patients in understanding and accepting medical diagnoses, procedures, and treatments.
- Provide counseling related to aging, discharge planning, and end-of-life issues.
Requirements
- Bachelor's degree in Social Work.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
