SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join LifeBridge Health in Baltimore, MD. As part of the Social Work team at Levindale Hospital, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various common issues including adjustment to institutional living, financial concerns, family dynamics, medical procedures understanding, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Address potential challenges related to patient care and treatment.
• Offer support and counseling to individuals and families during difficult times.
• Assist in discharge planning and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of social work experience.
• LBSW, LMSW, or LGPC certification.
• Knowledge of social work theory and practice.
