Job Description

We are looking for a passionate SOCIAL WORKER PRN to join our team at LifeBridge Health in Baltimore, MD. As a key member of the hospital's social work department, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of challenges from adjustment to institutional living to counseling related to death and dying.

Responsibilities

• Offer clinical social work services and psychosocial assessments.
• Address common problems such as adjustment to institutional living, financial concerns, and family relationships.
• Provide counseling on matters related to medical diagnoses, aging, and end-of-life care.
• Assist with discharge planning and empower patients and families facing challenging circumstances.

Requirements

• Basic professional knowledge, Bachelor's degree, and working knowledge of specialized field theory and practice.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
