SOCIAL WORKER PRN

Job Description

We are currently looking for a PRN Social Worker to join our team at LifeBridge Health. The successful candidate will provide clinical social work services and psychosocial assessments to patients, families, and residents, addressing various challenges and concerns. This role involves assisting individuals with adjustment to institutional living, financial issues, family dynamics, medical-related issues, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Deliver clinical social work services and psychosocial assessments.
• Address adjustment issues, financial concerns, family dynamics, and medical-related challenges.
• Provide counseling for end-of-life matters and discharge planning.

Requirements

• Bachelor’s degree in social work or related field.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
