Job Description
We are looking for a passionate Social Worker to join our team at LifeBridge Health. This position offers the opportunity to provide clinical social work services and psychosocial assessments to patients, helping them navigate through various challenges related to health, aging, and family dynamics.
Responsibilities
- Offer clinical social work services to patients and families, addressing a range of concerns including institutional living, financial challenges, and adjustment to medical conditions.
- Conduct psychosocial assessments and provide counseling on end-of-life issues and discharge planning.
- Collaborate with interdisciplinary teams to ensure comprehensive care for patients.
Requirements
- Bachelor's degree and 1-3 years of relevant experience.
- LBSW/LMSW/LGPC certification.
- Strong knowledge of social work theory and practice.
