Job Description
We are looking for a compassionate and dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, helping them navigate various challenges and life transitions. The position is based at Levindale Hospital in Baltimore, MD.
Responsibilities
- Conduct psychosocial assessments and provide counseling to address patient/resident and family needs.
- Assist in discharge planning and offer support related to death and dying.
- Collaborate with interdisciplinary teams to ensure comprehensive care.
Requirements
- Bachelor's degree in Social Work or related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
