SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients and their families, addressing a variety of common issues including adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address issues such as adjustment to institutional living, financial concerns, and discharge planning.
• Work collaboratively with a multidisciplinary team to meet the needs of patients and residents.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed Social Worker (LBSW/LMSW/LGPC).
