Job Description
We are looking for a dedicated and compassionate PRN Social Worker to join LifeBridge Health in Baltimore, MD. The position involves providing clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common issues related to institutional living, aging, family dynamics, medical procedures, and end-of-life counseling.
Responsibilities
- Conduct psychosocial assessments and provide clinical social work services.
- Assist patients and families in coping with various challenges and adjustments.
- Offer support in discharge planning and end-of-life counseling.
Requirements
- Basic professional knowledge with a Bachelor's degree; relevant experience preferred.
- LBSW/LMSW/LGPC certification.
- Compassionate and empathetic approach to patient care.
