SOCIAL WORKER PRN

Job Description

We are looking for a Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and support.
• Assist with discharge planning.
• Collaborate with the healthcare team.

Requirements

• Bachelor's degree.
• 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
• Strong knowledge of social work theory and practice.
