SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and experienced Social Worker on a PRN basis to join our team at LifeBridge Health in Baltimore, MD. In this role, you will provide clinical social work services and psychosocial assessments to patients, helping them navigate various challenges related to their health and well-being.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Address common problems such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
• Collaborate with interdisciplinary teams to support patient care.

Requirements

• Bachelor's degree and 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Knowledge of social work theory and practice.
