Job Description

We are looking for a compassionate Social Worker PRN to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services to patients and their families, addressing a variety of common issues related to healthcare, family dynamics, and end-of-life care.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support patients in adjusting to medical treatments and transitions.
• Address financial concerns, family relationships, and other challenges that impact patient well-being.

Requirements

• Bachelor's degree and LBSW/LMSW/LGPC certification.
• 1-3 years of relevant experience in a healthcare setting.
• Knowledge of social work theory and practice.
