Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. This position is on an as-needed basis with flexible hours. As a PRN Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients/residents and families, addressing various challenges related to institutional living, financial concerns, family dynamics, medical treatments, aging, discharge planning, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments and provide clinical social work services.
• Support patients and families in coping with emotional and practical aspects of healthcare challenges.
• Assist in discharge planning and facilitate discussions on issues related to aging and end-of-life care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed (LBSW, LMSW, or LGPC).
