SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and dedicated Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. In this PRN (as needed) role, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as institutional living adjustments, financial concerns, family relationships, and end-of-life counseling.

Responsibilities

• Conduct psychosocial assessments of patients and families.
• Provide counseling and support related to various emotional and practical challenges.
• Assist with discharge planning and end-of-life care discussions.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of experience.
• LBSW/LMSW/LGPC certification.
