Job Description

We are looking for a dedicated Social Worker PRN to join our team at LifeBridge Health. In this role, you will provide clinical social work services and psychosocial assessments for patients and their families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common problems including adjustment issues, financial concerns, and family relationships.
• Offer counseling related to medical procedures, aging, discharge planning, and end-of-life care.

Requirements

• Basic professional knowledge with a Bachelor's degree.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
