Job Description

We are looking for a dedicated PRN Social Worker to join the LifeBridge Health team at Levindale Hospital in Baltimore, MD. In this role, you will provide essential clinical social work services and psychosocial assessments to patients, residents, and their families, addressing a range of common challenges. From adjustment to institutional living to discharge planning and counseling for end-of-life decisions, you will play a crucial role in supporting individuals through difficult times.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address various challenges faced by patients and families.
• Assist with discharge planning and bereavement counseling.
• Collaborate with interdisciplinary teams to ensure holistic care.

Requirements

• Bachelor's degree with 1-3 years of experience.
• Licensed as LBSW, LMSW, or LGPC.
• Strong understanding of social work theory and practice.
• Compassionate and empathetic approach to patient care.
