Job Description
We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. In this role, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges. Responsibilities
- Offer clinical social work services and psychosocial assessments.
- Address various issues such as adjustment to institutional living, financial concerns, family relationships, aging, and end-of-life counseling.
- Assist with discharge planning and provide support related to medical procedures and treatments.
Requirements
- Bachelor's degree or equivalent with working knowledge of social work theories and practices.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
