SOCIAL WORKER PRN

Job Description

We are looking for a skilled Social Worker PRN to join our team at LifeBridge Health in Baltimore, MD. As a PRN Social Worker, you will be responsible for providing clinical social work services and psychosocial assessments to patients, residents, and their families, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.

Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist with discharge planning and support patients in adjusting to medical procedures.
• Collaborate with multidisciplinary teams to ensure comprehensive care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
• Strong communication and interpersonal skills.
