SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more. We are seeking a compassionate individual who is dedicated to helping others navigate difficult situations with empathy and support.

Responsibilities

• Provide psychosocial assessments to patients and families.
• Address common problems related to adjustment, financial concerns, and family relationships.
• Offer counseling and support for issues like discharge planning and end-of-life care.
• Collaborate with healthcare teams to ensure comprehensive patient care.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of experience in a similar role.
• Licensed as LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
