Job Description
We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. In this role, you will be providing clinical social work services and psychosocial assessments to patients, focusing on various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Provide clinical social work services to patients and families.
- Assess and address common psychosocial problems.
- Offer counseling related to death and dying.
- Conduct discharge planning.
Requirements
- Bachelor's degree and 1-3 years of experience.
- LBSW/LMSW/LGPC certification.
- Knowledge of social work theory and practice.
