SOCIAL WORKER PRN

Job Description

We are looking for a dedicated Social Worker on a PRN basis to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and counseling related to aging and end-of-life matters.

Responsibilities

• Offer clinical social work services to patients and families.
• Assess and address various psychosocial needs and challenges.
• Provide counseling on adjustment to medical conditions and end-of-life planning.
• Participate in discharge planning processes.

Requirements

• Bachelor's degree with 1-3 years of relevant experience.
• Licensed as LBSW, LMSW, or LGPC.
• Solid understanding of social work theory and practice.
• Compassionate and empathetic approach to patient care.
