SOCIAL WORKER PRN

Job Description

We are looking for a compassionate and skilled Social Worker to join our team at LifeBridge Health. This is a PRN position based in Baltimore, MD, at Levindale Hospital, with flexible hours as needed. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Conduct psychosocial assessments.
• Provide counseling and support to patients and families.
• Assist with discharge planning.
• Handle issues related to aging, financial concerns, and end-of-life care.

Requirements

• Bachelor's degree or equivalent in Social Work.
• 1-3 years of relevant experience.
• LBSW, LMSW, or LGPC certification.
