Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a vital member, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, and counseling related to death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Address issues related to adjustment to institutional living, financial concerns, and counseling for death and dying.
• Conduct discharge planning in collaboration with healthcare teams.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of experience in social work.
• LBSW, LMSW, or LGPC certification.
