Job Description
We are looking for a dedicated SOCIAL WORKER PRN to join our compassionate team at LifeBridge Health. As part of our Allied Health department, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and discharge planning.
Responsibilities
- Provide counseling and support to patients and families facing medical challenges and end-of-life care.
- Conduct psychosocial assessments to identify patient needs and develop appropriate care plans.
- Collaborate with healthcare professionals to ensure comprehensive care for patients.
- Assist in discharge planning and coordination of post-hospital services.
Requirements
- Bachelor's degree in social work or related field.
- 1-3 years of experience in clinical social work.
- LBSW/LMSW/LGPC certification.
- Strong interpersonal and communication skills.
