SOCIAL WORKER PRN

Job Description

We are looking for a PRN Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and families, addressing various common issues such as adjustment to institutional living, financial concerns, and discharge planning.

Responsibilities

• Offer psychosocial assessments and clinical social work services to patients and their families.
• Assist in addressing various challenges related to patient care and well-being.
• Provide counseling and support in areas like adjustment to medical procedures, aging, and end-of-life care.

Requirements

• Bachelor's degree in Social Work.
• 1-3 years of relevant experience.
• LBSW/LMSW/LGPC certification.
