SOCIAL WORKER PRN

Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and conduct psychosocial assessments for patients, addressing various issues such as adjustment to institutional living, financial concerns, family relationships, and more.

Responsibilities

• Deliver clinical social work services to patients/residents and their families.
• Conduct psychosocial assessments and address common problems related to medical conditions, aging, and end-of-life care.
• Assist with discharge planning and counseling for patients and families.

Requirements

• Bachelor's degree and 1-3 years of experience in social work.
• Licensed as LBSW, LMSW, or LGPC.
• Solid understanding of social work theory and practice.
