Job Description
We are looking for a skilled Social Worker to join our team at LifeBridge Health. This position, based in Baltimore, MD at Levindale Hospital, is offered on a PRN (as needed) basis from 8:30am to 5:00pm. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, assisting with various issues including adjustment to institutional living, financial concerns, family dynamics, medical procedures, aging-related changes, discharge planning, and end-of-life counseling.
Responsibilities
- Offer clinical social work services and assessments to patients and families.
- Address a range of psychosocial issues including financial concerns and family relationships.
- Assist patients in coping with medical treatments and procedures.
- Provide counseling on aging, powerlessness, discharge planning, and end-of-life matters.
Requirements
- Bachelor's degree in a related field.
- 1-3 years of relevant experience.
- LBSW, LMSW, or LGPC certification.
