Job Description

We are looking for a dedicated PRN Social Worker to join our team at LifeBridge Health. This position involves providing vital clinical social work services and psychosocial assessments to patients, residents, and families on various common issues concerning institutional living, financial matters, family dynamics, medical procedures, aging, discharge planning, and bereavement counseling. As part of our healthcare team, you will play a crucial role in supporting individuals through challenging life transitions and ensuring their well-being.

Responsibilities

• Offer clinical social work services to patients and families in a healthcare setting.
• Conduct psychosocial assessments and provide support for various challenges individuals may face.
• Assist with discharge planning and bereavement counseling.
• Collaborate with healthcare professionals to ensure comprehensive care for patients.

Requirements

• Bachelor's degree in Social Work or related field.
• 1-3 years of relevant experience.
• Licensed as an LBSW, LMSW, or LGPC.
• Strong communication and interpersonal skills.
• Compassionate and empathetic approach to patient care.
