Job Description
We are looking for a passionate Social Worker to join our Care Management team at Carroll Hospital, a part of LifeBridge Health. This part-time position requires a weekend commitment and involves day shifts from 9:00am to 5:30pm.
Job Summary
As a Social Worker, you will collaborate with the healthcare team to ensure efficient care for inpatients, address various needs such as abuse, neglect, substance abuse, mental health, and more. You will also coordinate with community agencies and link patients to essential resources.
Requirements
- Master's degree in social work.
- 2-4 years of recent social work experience, preferably in a medical setting.
- LCSW-C license preferred.
