Job Description

We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker for the BALTIMORE COUNTY/CITY region, you will play a vital role in providing professional and comprehensive support to individuals and families. You will collaborate with interdisciplinary team members to assess needs, formulate care plans, and ensure the delivery of high-quality services.

Responsibilities

• Assess psychosocial, environmental, and financial needs of patients and families.
• Develop and implement individualized care plans in partnership with patients and families.
• Serve as a consultant and educator to team members and patients.
• Ensure evidence-based practices and optimal patient care.

Requirements

• Master's Degree in Social Work from an accredited school.
• LCSW certification required.
• Excellent interpersonal communication skills.
• Ability to maintain accurate documentation.
