Job Description
We are looking for a dedicated Inpatient Case Manager (RN) to join our team at LifeBridge Health. This full-time position, based in Randallstown, MD at Northwest Hospital, offers a Sign-On Bonus Potential of $15,000. The role involves providing discharge planning coordination and intervention in collaboration with the clinical team and medical provider to promote patient wellness, improve care outcomes, optimize health service utilization, and minimize payment denials for patients with complex health needs.
Responsibilities

Requirements
- Bachelor's degree in nursing (BSN) required from CCNE accredited schools.
- Strong knowledge and practice within the field of inpatient care management.
- Prior experience in utilization review, clinical care coordination, or case management is desirable.
