Job Description
We are currently looking for a dedicated Social Worker to join the LifeBridge Health team. In this role, you will provide clinical social work services and psychosocial assessments to patients, families, and residents, addressing various social and emotional needs.
Responsibilities
- Conduct psychosocial assessments and provide counseling to individuals and families.
- Assist patients with adjustment to institutional living and coping with medical diagnoses.
- Develop discharge plans and provide support related to aging, death, and dying.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
