Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide vital clinical social work services and psychosocial assessments to patients, residents, and their families. Your role will involve addressing a range of common issues and challenges faced by individuals in healthcare settings.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address issues such as adjustment to institutional living, financial concerns, family relationships, and more.
- Assist with discharge planning and offer counseling related to end-of-life care.
Requirements
- Bachelor's in Social Work (BSW) required.
- Master's degree in Social Work preferred.
- LCSW-C certification preferred.
- LGSW certification required.
