Job Description
We are looking for a dedicated and compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges such as adjustment to institutional living, financial concerns, and counseling related to death and dying. You will play a crucial role in helping individuals navigate difficult situations and improve their overall well-being.
Responsibilities
- Provide clinical social work services and psychosocial assessments to patients and families.
- Address common problems such as adjustment to institutional living, financial concerns, and family relationships.
- Assist with discharge planning and counseling related to end-of-life issues.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
