Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services to patients/residents and families, addressing various challenges such as adjustment issues, financial concerns, family relationships, and end-of-life counseling. This full-time position offers day shifts from 8:00am-4:30pm.
Responsibilities
- Conduct psychosocial assessments and provide counseling services.
- Assist patients with discharge planning and transition to post-acute care.
- Collaborate with healthcare professionals to address patients' social and emotional needs.
- Advocate for patients' rights and access to resources.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Excellent communication and interpersonal skills.
