Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. In this full-time position based in Baltimore, MD, you will provide clinical social work services to patients and their families, addressing various challenges they may encounter during their medical journeys.
Responsibilities
- Conduct psychosocial assessments and offer support for adjustment issues.
- Assist with financial concerns and discharge planning.
- Provide counseling on family relationships and end-of-life matters.
- Collaborate with healthcare teams to enhance patient well-being.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C certification is preferred.
- LGSW certification.
