Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a key member of our Allied Health department at Levindale Hospital in Baltimore, MD, you will be responsible for providing clinical social work services to patients and their families. This role involves addressing a variety of challenges including adjustment to institutional living, family relationships, discharge planning, and counseling related to end-of-life care.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Assist patients with financial concerns and powerlessness issues.
- Provide counseling on medical treatments, aging, and family dynamics.
- Collaborate with interdisciplinary teams for effective care planning.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C certification preferred.
- LGSW certification.
