Registered Nurse - Intake & Hospital Liaison

Job Description

We are looking for a compassionate and dedicated Registered Nurse to join our Intake and Hospital Liaison team at UVM Health Network Home Health & Hospice. This crucial role involves serving as the primary point of contact between referring providers, program leaders, clinicians, and patients/families to facilitate seamless transitions to home health and hospice services.

Responsibilities

• Provide essential intake services, information, and education to patients, families, and providers.
• Coordinate patient discharges, review clinical documentation, and assess patient eligibility for services.
• Collaborate with interdisciplinary teams, attend rounds, and write orders for patient care.
• Ensure timely and effective communication to support a smooth care transition process.

Requirements

• Associate's degree in nursing and an active RN license.
• Minimum of two years of nursing experience.
• Strong interpersonal skills and the ability to work collaboratively with diverse stakeholders.
• Excellent clinical assessment and decision-making abilities.
