As part of the clinical team, you will be responsible for the evaluation and treatment of patients, developing outcomes-centered plans, and delivering effective care management to ensure timely and high-quality care for those we serve.

Responsibilities

• Evaluate and treat patients to establish outcomes-centered plans.
• Deliver effective care management across a patient's episode.
• Operate at the top of your license, appropriately leveraging and delegating care.
• Drive consistent and predictably positive outcomes for our patients.

Qualifications

• Licensed or license-eligible in the state of application.
