Job Description

The Licensed Practical Nurse at Hospice Of Hope, Inc provides one-to-one care at the bedside of terminally ill patients, based on an ongoing nursing assessment of need and in accordance with the established plan of care. This position involves three 12-hour shifts per week, including weekend rotation, and is eligible for a $5,000 bonus. Some of the key duties include:

  • Providing compassionate end-of-life care in accordance with the individualized plan of care, with demonstrated ability to assess and respond to patient and family needs in a timely manner.
  • Educating patients and caregivers on end-of-life services and other resources.
  • Documenting complex medical information using clinical tools, data, and other information to complete assessment of patient needs.
  • Establishing professional and collaborative relationships with patients, families, and providers utilizing strong written and verbal communication skills.
  • Having basic computer skills, including working knowledge of the Microsoft Office suite.
  • Working the night shift from 8:00 pm - 8:00 am, three (3) shifts per week, including every other weekend.
  • Providing care in private residences, nursing homes, assisted living facilities, in Kentucky and Ohio.
  • Earning a sign-on bonus of $2,500 after six months of full-time employment, and $2,500 after one year of full-time employment.

Professional and Personal Requirements

Graduation from an accredited School of Nursing with current licensure, or eligibility to be licensed, in Kentucky and Ohio is required. At least one year of experience in a patient care position in a hospital, nursing home, or related health care facility is preferred. Personal qualities of empathy, responsibility, and dependability are essential. Effective communication skills with team members, patients, providers, and family members are required. The ability to work independently of direct on-site supervision and demonstrate leadership and teaching ability is necessary. Additionally, the role involves physical requirements including being able to travel to client homes, nursing homes, hospitals, and other facilities independently, proper use of personal protective equipment, observation of universal precautions, and being able to lift at least 50 pounds using proper body mechanics. Must have a valid driver's license, current auto insurance, and reliable transportation.
