We are hiring a Home Health Registered Nurse (RN) to join Interim HealthCare of West Palm Beach. Responsibilities

• Deliver home-based care to patients with various medical conditions, post-hospitalization or discharge.
• Coordinate with physicians and supervise a team providing patient care.
• Conduct patient assessments, document progress, and communicate changes to the medical team.
• Educate patients and family members on treatment plans, medications, and home care practices.
• Ensure care goals are achieved and oversee the discharge process.

Requirements

• Minimum of 1 year of acute care experience, with a preference for home healthcare experience.
• Active RN license in the state of Florida and CPR certification.
• Familiarity with state and federal home health regulations.
• Strong clinical skills, effective communication, and compassion towards patients.
