Job Description
We are looking for a motivated Complex Care Social Worker - LMSW to join our team at LifeBridge Health in Baltimore, MD. As a Complex Care Social Worker, you will collaborate with the interdisciplinary team to ensure efficient and effective discharges in alignment with length of stay goals. Your responsibilities will involve using an escalation protocol to address barriers, monitor patient progress, and identify early intervention opportunities. Requirements
- Seasoned professional knowledge with a Master's degree.
